The Return-to-Duty Step 5 drug test is a federally required DOT test that must be completed after a driver violates DOT drug or alcohol regulations - and before returning to any safety-sensitive duties.
This requirement is mandated under 49 CFR Part 40 and enforced through the FMCSA Drug & Alcohol Clearinghouse. A driver cannot legally operate a commercial motor vehicle until the RTD test result is verified negative and properly reported to the Clearinghouse. Passing the test alone is not enough - the Clearinghouse update must also be completed before the driver may return to duty.
The RTD Step 5 test is required after any of the following violations:
Before ordering Step 5, the driver must complete the SAP evaluation and any required education or treatment. goMDnow does not provide SAP assessments. Step 5 should be ordered only after the SAP has made the driver eligible for RTD testing.

The observed collection is required by DOT for all RTD and follow-up tests - a same-gender collector must be present during the entire specimen collection.
SAP Assessment (Steps 1-4) is not included. goMDnow is not responsible for SAP assessments. The SAP process must be completed with an independent DOT-qualified Substance Abuse Professional before the RTD Step 5 test can be ordered.
Step 5 is the single return-to-duty test required before the driver can return to safety-sensitive work. Step 6 is follow-up testing that begins after the driver has returned to duty and follows the SAP-prescribed follow-up testing plan.

Employers and owner-operators share responsibility for ensuring the RTD process is followed correctly. An employer who allows a driver with PROHIBITED status to operate a CMV faces serious FMCSA penalties - even if the driver initiated the RTD process.
